A vague referral makes it harder for any provider to evaluate fit. A strong referral explains what is happening now, which risks matter, who is involved, and what outcome the care team is trying to support.
Define the actual referral question
“Needs home care” is broad. A referral becomes more useful when it explains whether the issue is unsafe daily routines, behavioral instability, cognitive deficits, workforce displacement, an unsuccessful residential setting, or another defined problem.
Include the information that changes the plan
Relevant diagnoses, injury history, mobility, current function, behavioral triggers, cognitive needs, physician or therapy recommendations, guardianship, existing providers, and prior placements can all influence what services deserve consideration.
Separate regional program availability
Focused Home Care has different service offerings in Georgia and Michigan. Referrals should identify the state and program rather than assuming every service exists in both markets.
Agree on communication expectations
The referral source should know who will receive updates, which changes require escalation, and how the provider will communicate with guardians, case managers, clinicians, or other professionals.
